The Role
Hours: Full time hours 37.5+ per week.
Role: Nurse Community Case Manager
Contract type: Permanent & Full time

Pharmakia is currently seeking a dynamic candidate with a valid UK driving license and own vehicle for a key role covering Lancashire, for a highly reputable and specialist client in complex care.
Extensive support and training is provided.
Essential:
UK Driving License, own vehicle, willingness to commute.
Desirable:
Tracheostomy and ventilation experience.

Ensure safe care, empower client choices, deliver excellent customer service, foster service growth, and consistently strive for excellence.
